1. Recite the blessing:


Blessing for the counting of the omer
   Baruch atah adonay eloheynu melech ha'olam asher kidshanu
   bemitzvotav vetzivanu al sefirat ha'omer

Blessed are you, ETERNAL, our God, the sovereign of all, who has
  made us holy with your mitzvot
  and commanded us concerning the counting of the Omer.

2. Count the Homer:

 

Hayom shmonah-asar yom, sh'hem shnei shavuot v'arba'a yamim la'omer.
Today is the eighteenth day, making two weeks and four days of the Omer.
